Periauricular Contouring Suspension in Secondary Face-Lift.

Abstract
BACKGROUND: The management of revision face-lift surgery is always challenging and controversial. We introduce the periauricular contouring suspension which combines a concentric SMAS suspension with a linear platysmal suspension for optimal facial rejuvenation. METHODS: Complete clinical examination was a prerequisite to any secondary face-lift. We included every patient who had undergone a secondary face-lift between 2007 and 2010 in this study. The periauricular contouring suspension technique was used. It is inspired by Stocchero’s round-block SMAS treatment procedure and also features a linear platysmal suspension associated with liposuction. Data gathered for every patient included sex, age, delay between primary and secondary procedures, and follow-up. RESULTS: Sixty-two patients (50 women and 12 men) underwent the procedure and were included in this study. The average age was 66 years old. The average delay between primary and secondary surgery was 11 years. Average follow-up was 29 months. CONCLUSIONS: Periauricular contouring suspension is a reliable technique for a secondary face-lift. It is a minimally SMAS-invasive technique in which postoperative outcomes are excellent and has become our preferred technique for revision face-lifting. Abstract
BACKGROUND: Extravasation injury is a common complication of neonatal intensive care and can result in scarring with cosmetic and functional sequelae. A wide variety of treatments are used in practice including subcutaneous irrigation with saline (with or without hyaluronidase), liposuction, use of specific antidotes, different topical applications and normal wound care with dry or wet dressings. All such treatments aim to prevent or reduce the severity of complications.
OBJECTIVES: To determine the efficacy and safety of saline irrigation or saline irrigation with prior hyaluronidase infiltration on tissue healing in neonates with extravasation injury when compared to no intervention or normal wound care.
SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2011, Issue 2), MEDLINE (1950 to June 2011), EMBASE (Jan 1980 to June 2011), CINAHL (Jan 1988 to June 2011) and the Web of Science (up to July 2011).
SELECTION CRITERIA: Randomised controlled trials (RCT) and quasi-randomised controlled trials comparing saline irrigation with or without hyaluronidase infiltration with no intervention or normal wound care in the management of extravasation injury in neonates.
DATA COLLECTION AND ANALYSIS: Three review authors independently reviewed and identified articles for possible inclusion in this review.
MAIN RESULTS: No eligible studies were found. There were a few case reports and case series describing successful outcomes with different interventions in this condition.
AUTHORS’ CONCLUSIONS: To date, no randomised controlled trial is available that examines the effects of saline irrigation with or without prior hyaluronidase infiltration in the management of extravasation injury in neonates. Saline irrigation is a frequently reported intervention in the literature that is used in the management of extravasation injury in neonates. Research should be initially directed at evaluating the efficacy and safety of this intervention through randomised controlled trials. It will also be important to determine the size of the effect according to timing of intervention, nature of the infusate and the severity of injury at the time of intervention.

High rate of solitary sentinel node metastases identification by fluorescence-guided lymphatic imaging in breast cancer.

Abstract
BACKGROUND: The lymph node status was shown to be an important prognostic factor for breast cancer, but controversial issues remain. There has been increased focus on optimizing the visualization of lymph nodes for an accurate and selective approach to axillary lymph nodes. Fluorescence-guided lymphatic imaging is a potential candidate for further research on remaining controversies.
METHODS: Forty-seven patients were subject to injection of indocyanine green for navigation to the SLN based on fluorescent dye retention detection. In two groups, patients either received intended axillary lymph node dissection (ALND) and sentinel lymph node biopsy (SLNB) for superstaging or isolated SLNB for minimal-invasive staging. The prospective study was designed to evaluate the technical feasibility with focus on solitary positive SLN.
RESULTS: Visualization of lymphatic vessels with bright fluorescence of the SLN was feasible in 46 of 47 patients. Eighteen of 19 nodal positive patients were correctly identified with a sensitivity of 94.7% in all patients after ALND. After immunohistochemistry, in 19 of 25 overall nodal positive patients (76%) the SLN was the only positive lymph node.
CONCLUSION: Fluorescence-guided imaging using fluorescence retention detection allows transcutaneous navigation with a high rate of solitary positive SLN identification as an alternative technique for further research.
