Background: Patients with syndromic craniosynostosis and midface hypoplasia are often treated with Le Fort III advancement. The authors present four patients with extraordinarily long-term follow-up (in excess of 20 years).
Methods: An institutional review board?approved retrospective chart review was performed on all patients with syndromic craniofacial synostosis who underwent Le Fort III advancement. Patients with greater than 20 years of cephalometric and photographic records were identified. Lateral cephalograms were obtained preoperatively, immediately postoperatively, at 1-year follow-up, and at long-term follow-up. Cephalograms were traced, digitized, and averaged. Fifty cephalometric landmarks were identified for serial measurements.
Results: Of the four patients identified, one had Apert syndrome and three had Crouzon syndrome. Average age at the time of Le Fort III advancements was 11 years (range, 4 to 20 years). Average length of postoperative follow-up was 25 years (±5 years). No patient had significant anterior midfacial growth following Le Fort III advancement. Both young patients (<10 years) had substantial vertical inferior midfacial growth after advancement.
Conclusions: These data demonstrate that the Le Fort III segment of children with syndromic craniosynostosis does not grow significantly forward. Moreover, the traditional Le Fort III osteotomy does not provide the amount of midface advancement necessary to avoid phenotypic recidivism in these syndromic patients. This study also suggests that patients undergoing Le Fort III advancement appear to have zygomatic effacement and ptosis of the overlying soft tissue with deepening of the facial folds; collectively, it is suggested that these changes give the appearance of accelerated facial aging.

Brava® and Autologous Fat Transfer Is a Safe and Effective Breast Augmentation Alternative: Results of a Six-Year, Eighty-One Patients Prospective Multicenter Study.

Abstract
INTRODUCTION:: Breast augmentation by autologous fat transfer is an appealing alternative in need of scientific validation. MATERIALS AND METHODS:: A prospective multicenter study enlisted 81 women (age 17-63). Patients wore Brava®, a bra-like vacuum-based external tissue expander, for four weeks, then underwent autologous fat injection using 10 – 14 needle puncture sites into each breast in a 3D fanning pattern (average, 277 ml volume injected per breast). Patients resumed Brava® wear within 24 hours for 7 or more days. Pre- and post-treatment breast volumes were derived from 3D volumetric reconstruction of MRI scans, and outcomes were compared to a meta-analysis of 6 recent published reports on AFT breast augmentation without expansion. Follow-up ranged from 12 months to 6 years (average 3.7 years). RESULTS:: Breast volume was unchanged between 3 and 6 months. Seventy-one of the treated women were compliant with Brava wear and had a mean augmentation volume at 12 months of 233 ml/breast compared to 134 ml/breast in published series without Brava (p<0.00001). Graft survival was 82 ± 18% compared to 55 ± 18% without Brava (p<0.00001). There was a strong linear correlation (R = 0.87) between pre-grafting Brava expansion and the resultant breast augmentation. There were no suspicious breast masses or nodules. MRI recognized a 16% incidence of fat necrosis easily identified at one-year mammographic evaluation. SUMMARY/CONCLUSIONS:: The addition of Brava® expansion prior to autologous fat grafting leads to significantly larger breast augmentations, with more fat graft placement, higher graft survival rates, and minimal graft necrosis or complications, demonstrating high safety and efficacy for the procedure.

Background: The advent of self-inflating hydrogel tissue expanders heralded a significant advance in the reconstructive potential of this technique. Their use, however, is limited by their uncontrolled isotropic (i.e., uniform in all directions) expansion.
Methods: Anisotropy (i.e., directional dependence) was achieved by annealing a hydrogel copolymer of poly(methyl methacrylate-co-vinyl pyrrolidone) under a compressive load for a specified time period. The expansion ratio is dictated by the percentage of vinyl pyrrolidone content and the degree of compression. The expansion rate is modified by incorporating the polymer within a silicone membrane. The in vivo efficacy of differing prototype devices was investigated in juvenile pigs under United Kingdom Home Office Licence. The devices were implanted within a submucoperiosteal pocket in a total of six porcine palates; all were euthanized by 6 weeks after implantation. A longitudinal volumetric assessment of the expanded tissue was conducted, in addition to postmortem analysis of the bony and mucoperiosteal palatal elements.
Results: Uncoated devices caused excessive soft-tissue expansion that resulted in mucoperiosteal ulceration, thus necessitating animal euthanasia. The silicone-coated devices produced controlled soft-tissue expansion over the 6-week study period. There was a statistically significant increase in the volume of expanded soft tissue with no evidence of a significant acute inflammatory response to the implant, although peri-implant capsule formation was observed. Attenuation of the bony palatal shelf was noted.
Conclusion: A unique anisotropic hydrogel device capable of controlled expansion has been developed that addresses a number of the shortcomings of the technology hitherto available.

Comparing Breast-Reduction Techniques: Time-to-Event Analysis and Recommendations.

Abstract
BACKGROUND: Breast reduction is a common procedure used to improve physical and aesthetic factors associated with breast hypertrophy. This study investigated how surgical technique alone affects the risk factors for complications and profiled differences between techniques. Complications were assessed by the use of time-to-event methods. METHODS: Patient information was extracted from a cohort of 283 patients. Demographic, surgical, and follow-up information was analyzed for patients undergoing surgical procedures using the inferior pedicle Wise pattern (IPWP) and modified Hall-Findlay (MHF) techniques. The patients managed with the IPWP technique were considered control subjects. The failure rates were described using the Kaplan-Meier failure estimator to provide a true estimate of the experienced complication rates. RESULTS: Overall, few differences were noted between the groups except for total tissue removed. The overall failure (complication) rate at 6 months was 18.8%, with 9% of all the patients experiencing a major complication that required operative intervention/revision. As expected, the period with the greatest risk of complication was the first month after surgery. Surgical technique, total tissue removed, and age were nonpredictive of complications. Overall, the IPWP group had significantly more total tissue removed than the MHF group (median difference, 227 g; P = 0.002). There was no evidence of a learning curve when an experienced surgeon moved from the one technique to the other. CONCLUSION: At 6 months after surgery, 19% of patients are expected to have experienced a complication. There appears to be few differences in outcomes between the techniques of breast reductions used, and the success or otherwise almost certainly relates to factors independent of surgical technique and includes patient selection, operative skill, and experience. Time-to-event analysis provides a precise assessment and description of the complication profile. LEVEL OF EVIDENCE III: This journal requires that authors assign a level of evidence to each article. Background: This article examines outcomes following repeated breast reduction using vertical scar reduction mammaplasty. The results of performing repeated breast reduction in patients for whom operative records were available for the previous breast reduction were compared with those for whom these records could not be obtained.
Methods: A retrospective review of all patients who underwent repeated breast reduction for recurrent symptomatic mammary hypertrophy, inadequate volume reduction during the primary operation, and significant postoperative breast volume asymmetry was performed.
Results: Twenty-five patients had repeated breast reduction. The initial technique was known in 13 patients and unknown in 12 patients. The average total reduction per breast (including liposuction) was 658 g (range, 30 to 1150 g). Liposuction was used more often in cases for which the initial technique was unknown (p = 0.000). No patients experienced necrosis of the nipple-areola complex, and there was no significant difference in the complication rates between patients for whom the previous pedicle was known versus those in whom it was unknown (p = 0.220).
Conclusions: Using vertical scar reduction mammaplasty, repeated breast reduction is a safe procedure, even when the initial technique is unknown. A vertically oriented, inferior wedge excision of tissue can be safely excised, irrespective of the initial pedicle. For patients with ptosis in whom the nipple-areola complex needs to be transposed superiorly, a carefully planned and de-epithelialized superior pedicle should be used. In addition, liposuction is an important adjunct to achieve volume reduction, while limiting the amount of dissection during repeated breast reduction.

Abstract
BACKGROUND: Ptosis with excess skin in the pubic area is a very common deformity in patients after massive weight loss. This deformity requires adequate surgical treatment whether combined with abdominoplasty or not. The enlarged pubogenital area may lead to psychosocial distress and impaired quality of life. METHODS: A series of 23 women with a mean age of 39.5 years who previously underwent bariatric surgeries and later presented with pubogenital ptosis underwent monsplasty. The preoperative surgical markings and the surgical technique presented are easily reproducible. In this prospective study, the surgical outcomes were assessed by questionnaires applied to the patients, who scored the following parameters: movement dynamics, aesthetic appearance, sexual performance, improved hygiene, and use of clothing items. RESULTS: Four of the parameters assessed (movement dynamics, aesthetic appearance, hygiene, and use of clothing items) showed clear improvement, with scores ranging from good to very good. A small percentage of the patients (13%) reported fair improvement in sexual performance. CONCLUSION: The findings showed monsplasty to be a simple and reproducible technique with favorable outcomes and low morbidity rates.
